In order that St. James Church may be faithful in its stewardship of the funds entrusted to it by its members, this form has been prepared for use by those individuals seeking support for short term missions (1 day to 1 month). Please complete this form in its entirety. This form is to be used for requests made to the St. James Church Missions Team. Once it is submitted it will be reviewed by the team and you will be informed of whether or not your request has been approved or not.
By signing and submitting this form, you are acknowledging that the information above is true and accurate. Any fraudulent requests may result in legal action. If the trip is cancelled, it will be my responsibility to return the funds to St. James Church within 30 days of my scheduled trip.
